There are lot of things involved in design of reinforcements. So you have to follow those rules. there is no one rule. basic guide lines are given in the code. If you follow a code you can do it easily.

The basic theory of concrete reinforcing is that Steel bars are very good in tension and it gives sufficient amount of warning before failure. Reinforcement of steel bars make the concrete to bear tensile load also along with compression.
